Vendor Registration

Thanks for your interest in becoming a preferred CMS vendor.  Please complete the application form and provide the required information.  Each application will be reviewed by the CMS Vendor Review Committee (CVRC). Approved applicants will be notified by email and will be added to the Preferred Vendor List upon approval. Applications that are not approved will be kept on file for future consideration.
